Person Centred Health Care Assessments and the Development Of Health Care Plans Appraisal

This appraisal has been developed to indicate whether a nurse has increased their knowledge through the completion of the core standard. It has been designed to promote consistent and efficient best practice. It forms part of the supporting resources for the Core Standards Program developed by FACS.
