Hunter New England Homeless Youth Assistance Program (HYAP)
This service provides 24/7 access and support to young people who are homeless or at risk of homelessness in the Maitland, Tamworth and Armidale areas, and to partners supporting these young people.
Hunter New England HYAP provides comprehensive person-centred needs assessments, which allow for support to be tailored to each young person. Samaritans also works with a number of partners to ensure the development of case plans are client-centred and culturally appropriate.
Support to young people includes early intervention; intensive support in the home; family mediation; telephone support and referrals; assertive outreach; crisis and transitional accommodation; brokerage and advocacy; and intensive multidisciplinary support.
Follow up post-crisis support is also provided through ongoing case management, family mediation and support to prevent breakdown from occurring, and connections to localised support.
